Nonvascular pediatric interventional radiology practice is dominated by musculoskeletal interventions. Percutaneous bone biopsy using fluoroscopic or CT guidance is becoming more common in the diagnosis of both neoplastic and infectious diseases. As technology advances, therapeutic interventions are becoming more common. The combination of biopsy and radiofrequency ablation under imaging guidance for osteoid osteomas is well-documented and highly effective. With good results, a percutaneous biopsy of suspected histiocytosis lesions can be followed by direct injection of a steroid into the lesion. Preoperative embolization of aneurysmal bone cysts and tumors has reduced the morbidity associated with these operations significantly. Primary sclerotherapy is also used to treat aneurysmal bone cysts. Because the majority of these procedures are quite painful, general anesthesia is usually required, along with the same preprocedural preparation as for image-guided biopsy. The majority of these procedures can be completed in a single day.
